  • Any scans or imaging (Xrays, CT scans, MRIs etc)

  • Medicare card and private health insurance details

  • Workers compensation or Third party details

  • Any correspondence related to your condition

  • Any other relevant information, such as a list of your current medications

Fees:

A consultation fee will be charged. This will vary depending on initial or follow up consultation, or if your injury is associated with Workcover or CTP claim (motor vehicle accident).

If you require surgery, Dr Jenkins’ team will discuss with you the fees involved.

For Workcover or CTP patients, Dr Jenkins team will need your claim details to liaise with the insurance company for surgery approval.
